Location 1 - Adams Memorial Hospital  Hours are: M-F 7:30 am to 7:30 pm and Sat/Sun 9am to 4pm.  

Location 2 - Adams County Health Department 313 W Jefferson St, Decatur.  Hours are Monday 11:30 am to 2:30 pm, Tuesday and Wednesday 10 am to 2:40 pm, and Thursday 9 am to 3:20 pm.
